This Business Associate Agreement ("BAA") is between the customer named at checkout ("Covered Entity") and FlowClick, the provider of SolidifAi ("Business Associate").
1. Permitted uses. Business Associate will use and disclose protected health information only to provide the SolidifAi services and as the law requires.
2. Safeguards. Business Associate will keep administrative, physical and technical safeguards that protect the confidentiality, integrity and availability of protected health information.
3. Reporting. Business Associate will report any use or disclosure not provided for by this BAA, including breaches of unsecured protected health information, without unreasonable delay.
4. Subcontractors. Business Associate will make sure any subcontractor that handles protected health information agrees to the same restrictions.
5. Access, amendment and accounting. Business Associate will support the Covered Entity's obligations to give individuals access to, amendment of and an accounting of disclosures of their information.
6. Term and termination. This BAA lasts as long as the SolidifAi subscription. On termination, Business Associate will return or destroy protected health information where feasible.
